In the event that my Child/Children is injured whilst Training/Playing Football/Travelling to and from Football Matches/Events and I cannot be contacted on the above number(s), I hereby give my consent for my child to receive medical attention.

I agree to be bound by and to observe the Katanga Youth FC Rules and the Regulations of the Football Association Limited and Football Association, and all Competitions in which the Club Participate.

Photographic Consent: Images of members under the age of 18 will never be identified or named without parental consent and will only be used in accordance with the Clubs child protection/photography policies and Best Practice guidelines.

I hereby consent to Katanga Youth FC processing my/my child’s personal data which I have supplied for the purpose of managing membership. I understand that I have the choice not to grant my consent or, once given, to withdraw my consent at any time. I acknowledge that withholding or withdrawing my consent to process this data may restrict the services and activities available to me through Katanga Youth FC and may cause administrative delays. I understand information will be processed in line with Katanga Youth FC Data Privacy Notice.
